HIFU Contraindications: Who Should NOT Get This Treatment

HIFU (High Intensity Focused Ultrasound) is one of the safest and most effective non-invasive treatments for facial and body rejuvenation. The original Ulthera platform was FDA-cleared under 510(k) K121466, and peer-reviewed safety reviews (e.g., PubMed 27538219) confirm its favorable safety profile. Millions of sessions are performed worldwide each year with excellent outcomes. However, like any medical procedure, it is not suitable for everyone. A thorough medical evaluation before treatment is essential to ensure your safety and achieve the best possible results.

At Elyzea, Dr. Geldres — a board-certified plastic surgeon with extensive knowledge of facial anatomy — performs a personalized assessment before every treatment. Here is what you need to know before scheduling your session.

Absolute Contraindications

HIFU should not be performed in the following cases:

  • Pregnancy and breastfeeding: As a precaution, no focused ultrasound treatment is performed during pregnancy or breastfeeding. While there is no evidence of harm, the safety of the baby is the priority.
  • Metal implants in the treatment area: Plates, screws, or other metallic devices in the area being treated can interfere with ultrasound energy. Important note: hyaluronic acid fillers are NOT a contraindication — they are not metallic and do not interact with HIFU.
  • Open wounds, active infections, or severe active acne: The skin in the treatment area must be intact and free of infection. The session can be rescheduled once the skin has healed.
  • Pacemakers or implanted electronic devices: Cardiac devices and other electronic implants may be affected by focused ultrasound energy.
  • Severe blood clotting disorders: Patients with serious coagulation problems have a higher risk of bruising and complications.
  • Active cancer or ongoing oncological treatment: HIFU is not performed on patients with an active cancer diagnosis or those currently receiving chemotherapy, radiation therapy, or other cancer treatments.

Relative Contraindications (Require Medical Evaluation)

In these cases, Dr. Geldres will evaluate your individual situation to determine whether HIFU is safe for you:

  • Autoimmune diseases (lupus, scleroderma): Some autoimmune conditions can affect how the skin responds to treatment. Each case is evaluated based on disease activity and current medication.
  • Recent thread lifts: It is recommended to wait at least 6 months after thread lift placement before undergoing HIFU in the same area.
  • Recent dermal fillers in the area: If you received hyaluronic acid or another filler recently, we suggest waiting 2 to 4 weeks before your HIFU session.
  • Active rosacea or highly sensitized skin: Inflamed or hypersensitive skin may react excessively to the ultrasound heat. In some cases, the protocol can be adjusted, or we may wait until the skin has stabilized.
  • Isotretinoin (Accutane): If you are currently taking isotretinoin or recently completed treatment, it is recommended to wait at least 6 months before HIFU, as this medication alters skin healing and sensitivity.

Common Side Effects (Normal and Temporary)

HIFU is a safe treatment, but it is normal to experience some mild effects after your session. These are part of your skin's natural response and resolve on their own:

  • Mild redness: Similar to what appears after sun exposure. It typically fades within a few hours.
  • Mild swelling: Especially in sensitive areas such as the eye contour or jawline. It resolves within 24 to 48 hours.
  • Tenderness to the touch: The treated area may feel slightly sensitive for 1 to 2 days.
  • Tingling: A slight prickling sensation indicating collagen activation in the deep layers. It disappears within a few hours.

Rare Side Effects

  • Minor bruising: This may appear on very delicate skin or in areas with superficial blood vessels. It resolves within 5 to 7 days.
  • Temporary numbness: In rare cases, there may be a slight decrease in sensation in the treated area that recovers spontaneously within days or weeks.

Why Medical Evaluation Matters

HIFU may seem like a straightforward treatment — and in expert hands, it is. But the difference between an excellent result and a poor experience lies in who operates the equipment. Dr. Geldres, as a board-certified plastic surgeon, possesses a deep understanding of facial anatomy: the layers of the skin, the location of nerves and blood vessels, and the distribution of fatty tissue. This knowledge allows her to identify risks that non-medical operators might overlook.

At Elyzea, the pre-treatment evaluation is free and without obligation. If after the consultation we determine that HIFU is not the best option for you, we will recommend alternatives. It is always better to ask than to assume.

HIFU at Elyzea — what makes it deliver

HIFU (high-intensity focused ultrasound) only delivers its lifting effect when the energy reaches the SMAS layer at 4.5 mm with calibrated, predictable energy — and that requires a real platform (Ulthera/Ultherapy, Ultraformer III, or Doublo) plus an operator with the anesthesia infrastructure to run full energy without compromise. The Lima market is flooded with cheap "7D HIFU" Chinese clones marketed under similar-sounding names; these devices cannot reach SMAS depth reliably and produce inconsistent results. At Elyzea, the HIFU platform is genuine, the operator is experienced, and the anesthesia infrastructure (topical + optional nerve block or oral sedation) means we run full SMAS energy comfortably. Per the price list: HIFU full face S/1,000 (~US$286), face + neck S/1,500 (~US$429). Annual maintenance sustains the lifting result. For US patients, this is roughly 80–90% below typical US Ulthera pricing.

Why Elyzea is different in Lima

Three things separate Elyzea from most aesthetic-medicine providers operating in Lima and across Latin America. First, we use real, manufacturer-genuine equipment — not the Chinese imitations sold under similar-sounding brand names that flood the regional market. Whether the procedure involves a HIFU platform, a CO₂ laser, a Morpheus8 unit, a picosecond laser, or any other technology, the device is the genuine FDA-cleared (or equivalent) product, with manufacturer service contracts and original consumables. Second, an MD anesthesiologist is on-site for procedures that require it. Topical numbing alone is inadequate for many protocols at the depth or energy that actually delivers results; on-site anesthesia means we can run the protocol as designed without forcing patients to grit through pain or quietly reducing energy mid-procedure. Third, a full clinical setup includes a treatment room, an anesthesia bay with proper emergency equipment, a private recovery room where patients can decompress 30–60 minutes before leaving, and a dispensary that stocks post-procedure care products. Not a single-bed spa room.

For patients flying to Lima from the US, Canada, Europe, or other Latin American countries, this clinical infrastructure is the prerequisite — not a marketing add-on. When you're a medical tourist, you don't have a local primary-care doctor and you don't have family in the same time zone. The clinic itself has to be capable of handling whatever happens during and after the procedure, in real time.
