Botox Aftercare: First 24 Hours Do's and Don'ts

← Back to Blog

The first 24 hours after botulinum toxin matter more than people think. The product needs to settle into the targeted muscle without diffusing. A few simple do's and don'ts protect your investment and prevent asymmetry.

DO

  • Stay upright for 4 hours minimum
  • Drink plenty of water
  • Make exaggerated facial expressions for the first hour (helps the toxin bind to the muscle)
  • Apply mineral SPF 50+
  • Eat a meal before treatment to prevent lightheadedness
  • Take photos for before/after comparison

DON'T

  • Lie down for 4 hours
  • Rub or massage the treated areas for 24 hours
  • Wear tight-fitting hats or sunglasses pressed on the brow for 4 hours
  • Drink alcohol for 24 hours (increases bruising)
  • Exercise vigorously for 24 hours
  • Get facials, microneedling, or any other treatment for 1 week
  • Sleep face-down on the first night
  • Take aspirin or ibuprofen 24 hours before/after

When to call the clinic

  • Eyelid drooping within 1–2 weeks (rare; usually transient)
  • Asymmetric expression that persists past day 14
  • Severe headache that doesn't resolve
  • Allergic reaction signs (hives, swelling, breathing difficulty)
  • Vision changes
  • Difficulty swallowing

When toxin kicks in

Most patients see effects starting at day 3, full effect at day 14. Don't judge results before day 14. If asymmetry remains at day 14, request a touch-up — most reputable clinics include this.

If you have a specific event in mind, allow at least 14 days between treatment and event for full effect.

Why these rules matter

Lying down too soon: theoretical risk of toxin migration to unintended muscles (e.g., levator palpebrae causing eyelid droop).

Massaging treatment area: pushes toxin to unintended sites.

Vigorous exercise: increased blood flow may affect uptake.

Tight hats: pressure on treated frontalis.

What you can do same day

  • Light walking
  • Eating normally
  • Working at desk
  • Driving
  • Light conversation
  • Hydration
  • Reading, screens

Maintenance cadence

Every 4–5 months for optimal effect. Going 6+ months allows the muscle to fully reactivate, which can cause faster wear-off on subsequent treatments.

Consistent quarterly cadence produces best long-term results.

Common mistakes

  • Doing dental work day-of (jaw movement)
  • Going to gym after work
  • Sleeping in tight-fitting elastic headband
  • Touching face frequently when nervous
  • Aggressive facial massage at home

Frequently asked questions

What if I accidentally lay down before 4 hours?

Brief 5-10 minute lying down is generally fine. Extended sleep within 4 hours theoretically problematic but real-world significance unclear.

Can I shower?

Yes after a few hours. Avoid hot water on treated area.

Can I wear contact lenses?

Yes.

What about my normal skincare?

Resume normal skincare after 24 hours. Pause retinoids 1 week post-treatment.

Should I avoid kissing?

Avoid intense pressure on lip area for 24 hours after lip filler. No restriction after toxin only.

Can I get on a plane?

Detailed flying guide.

What about the gym?

Skip 24 hours. Light walking fine.

Bottom line

First 24 hours: stay upright 4 hours, no massage, no exercise, no alcohol. Effects start day 3, peak day 14. Touch-up at 2-week follow-up for any asymmetry.
