The first 30 days after fractional CO₂ laser determine whether the result is excellent or merely acceptable. The skin is rebuilding both its surface barrier and its collagen scaffolding during this window, and the products you apply (or don't) matter more here than at any other time. This is the complete 30-day aftercare protocol used for Elyzea CO₂ patients in Miraflores, Lima.

Why aftercare matters more than most patients realize

The CO₂ laser is the controlled wound. The 30-day aftercare is the ideal healing condition — and how you stack those conditions decides the outcome. Two patients with identical procedures by the same physician can end up with very different results based on aftercare adherence. The single biggest variable is sun exposure, followed by product irritation and product timing.

The four phases of 30-day recovery

The protocol breaks into four phases:

  • Phase 1 (days 0-3): wound stabilization — barrier cream, no actives, gentle cleansing
  • Phase 2 (days 4-10): re-epithelialization — barrier cream tapers, SPF starts, gentle moisturizers
  • Phase 3 (days 11-21): healing surface — reintroduce hydrating actives, no exfoliants yet
  • Phase 4 (days 22-30): normalization — reintroduce gentle vitamin C and gentle retinol; full cosmetic routine resumes around day 30-35

Days 0-3: barrier protection

The skin is open at this stage. Anything you apply goes deep. Use only:

  • Lukewarm water cleanse twice daily (no cleanser yet, or a fragrance-free non-foaming cleanser if needed)
  • Cicaplast B5, Avène Cicalfate, or equivalent barrier repair cream — every 2-3 hours
  • Thermal water spray as needed for heat relief
  • No SPF yet — stay indoors and away from windows

Avoid: fragrance, alcohol, retinoids, exfoliants, vitamin C, niacinamide above 5%, anything with active ingredients listed beyond glycerin and ceramides.

Days 4-10: SPF starts, barrier continues

By day 4-5, the surface starts closing. Add:

  • Mineral SPF 50+ every morning, reapplied every 2 hours if outdoors
  • Continue barrier cream 3-4 times daily
  • Switch to a fragrance-free gentle cleanser (Cetaphil, La Roche-Posay Toleriane)
  • Hyaluronic acid serum (5%) is OK starting day 7 if you tolerate it

Avoid: any sun exposure beyond brief, covered errands. No swimming pools (chlorine), no beach, no hot tubs, no saunas.

Days 11-21: gentle additions

The surface is intact and the deep healing continues. Add:

  • A gentle moisturizer with ceramides (CeraVe PM, La Roche-Posay Toleriane Sensitive)
  • Niacinamide 5% in the evening — supports barrier function
  • Continue mineral SPF 50+ every morning, religiously

Still avoid: retinoids, AHAs, BHAs, vitamin C above 5%, scrubs, washcloths, exfoliating tools.

Days 22-30: gradual reintroduction of actives

By week 4 the skin tolerates more. Cautious reintroduction:

  • Vitamin C 10% (L-ascorbic acid) in the morning — start every other day, build to daily
  • Retinol 0.25-0.5% in the evening — start once a week, build to 2-3 times a week by week 6
  • Continue niacinamide and ceramides
  • Mineral SPF 50+ continues daily through month 3 minimum

Still avoid: anything stronger than retinol 0.5% (no tretinoin yet), no AHA/BHA exfoliants, no physical scrubs.

Sun protection is non-negotiable

Lima's coastal grey skies are deceptive — UV index regularly hits 7-10 even when overcast. Mountain altitude during travel multiplies UV exposure. The CO₂ healing skin is uniquely vulnerable to post-inflammatory hyperpigmentation triggered by UV. The protocol:

  • Mineral (zinc oxide-based) SPF 50+ every morning, reapplied every 2 hours if outdoors
  • Wide-brim hat outdoors
  • UPF 50+ clothing or scarves around the face during prolonged outdoor exposure
  • Avoid the beach for 3 months minimum
  • Avoid hiking at altitude (Cusco, Machu Picchu, the Andes) for 3 months — UV doubles per 1,000 m elevation
  • Tinted SPF helps if you'll be in light makeup

Common aftercare mistakes to avoid

  • Picking crusts before they release naturally
  • Reintroducing actives too early ("I feel fine on day 14")
  • Using a chemical sunscreen instead of mineral during recovery (chemical can sting irritated skin)
  • Skipping SPF on overcast days
  • Returning to the gym sauna at day 7
  • Trying a new brand of moisturizer mid-recovery — stick with what your physician recommended
  • Drinking heavily during peak inflammation (alcohol slows healing and worsens swelling)

Frequently asked questions

Can I exercise during the 30 days?

Light walking from day 4. Moderate exercise from day 7-10. Avoid heavy sweating until day 10. Avoid steam, sauna, and hot yoga for 4 weeks.

When can I get a facial again?

Hands-off facials (LED, light hydrating) at week 4-6. Extractions, peels, microdermabrasion: 8-12 weeks minimum.

Can I have alcohol during recovery?

Skip days 0-3 entirely. Moderate consumption from day 4 is OK; heavy drinking should be avoided through week 4 — alcohol is inflammatory and slows healing.

Should I take supplements?

Vitamin C, zinc, and adequate protein support wound healing. Avoid anti-inflammatory supplements (turmeric, fish oil at high doses) for 1 week pre-treatment if you're not on prescribed medications.

What if I have a flight scheduled day 7?

Cabin air is dry and irritating. If unavoidable, bring barrier cream and thermal water spray onto the flight, apply hourly. Mineral SPF for the airport. Most patients prefer to fly home day 8-10 minimum.

Can I do dental work during the 30 days?

Routine cleaning is fine. Avoid major dental work or facial-pressure procedures for 2-3 weeks.
